We are strong believers in the value of preventative health measures.  GPs are experienced in preventing serious conditions before they occur.  As part of our program, we offer screening to for conditions such as diabetes, heart disease and common cancers.  We can advise and implement measures and reduce your risk and keep you healthy.

 

Common Preventative Health Consultations include:

  • Cardiovascular Risk reduction (stroke and heart disease)
  • Cholesterol and Blood pressure reduction

GP Management Plans at Newtown Medical Centre

What is a ‘Care Plan’? Care plans, also known as GP management plans, are specific appointments designed to manage chronic health problems. The aim is to identify and improve your chronic medical conditions through a structured plan and clear actions.

Benefits of Care Plans:

  • Collaboratively identifies your health goals and develops actionable steps for your overall well-being.
  • Provides accountability and follow-up outside of your regular appointments.
  • Involves our experienced nurses in tracking and monitoring your health.
  • You may be eligible for up to 5 allied health appointments attracting a Medicare rebate, helping you achieve your health goals.

What to Expect: Our nurse will call you prior to your appointment to prepare the care plan, including:

  • Identifying your chronic conditions of focus.
  • Collaboratively working on your goals and actions.

We then send a copy to those involved in your care (specialists and allied health) so that everyone can work towards achieving your goals. Your GP will review and finalize the plan during a face-to-face or telehealth appointment.

Follow-Up and Accountability: The most important part of your care plan is follow-up and accountability. Plans are reviewed by our nurses every 3 months via a face-to-face or telehealth appointment to update, alter, and improve your plan. This ensures you are achieving better health outcomes.

No Out-of-Pocket Cost: The best part is that we provide this service without any out-of-pocket cost, as we understand the value of improving your health.

Approved Allied Health Providers: Approved providers include physiotherapists, podiatrists, dietitians, exercise physiologists, occupational therapists, and others.

GP Health Assessment for Those Over 75

Did you know that anyone over the age of 75 is entitled to a comprehensive medical health assessment at no out-of-pocket cost? Newtown Medical Centre is pleased to provide this valuable service to you, and our GPs recommend you do this yearly!

What to Expect:

  • A 45-minute session with our nurse, followed by an appointment with your own doctor.
  • A proactive focus on keeping you independent and safe in your home.
  • Assistance with accessing necessary supports and services.

Benefits:

  • This assessment offers you and your doctor the opportunity to address any issues that may not have been apparent during routine appointments, which can sometimes be limited by time or acute issues.
  • Ensure you are up to date with recommended preventative health measures such as vaccination programs and cancer screening.
  • Link with our other recommended proactive services such as GP management plans or home medicine reviews.
